Support and Choice aims to provide a range of supports to individuals with a disability, their families, caregivers and communities to achieve their goals and aspirations.
Support may include:
- Direct individual support in the home to maximise independence
- Access to essential aids and equipment
- Case management or respite support
- Social and recreational support in the community
Individual Planning
The planner works with the person with a disability and their support network to explore access to informal, formal and specialised disability supports a develop a detailed plan specific to the individual.
The Support and Choice program includes the development of new skills or assistance with:
- Personal care - Bathing, hygiene, toileting, grooming, dressing
- Community access - Participating in local community recreation, leisure and cultural activities, utilising community facilities and services such as banks, shops and libraries and interacting with the others in the community.
- Other supports - Household management tasks such as cooking, menu planning, laundry, managing household finances, decision making, day-to-day problem solving and conflict resolution.
